Intake Instructions
Call 1-800-848-8327 between 8 am and 5 pm M – F with Crisis Assessment information and let the person who answers know you are calling about a pre-purchased appointment and need to speak to a care advocate.
Intake Process
Brief assessment over the phone; appointment usually scheduled within two weeks (may be sooner); info will be faxed to clinic for review.
Service Description
Provides access to medication management services, as well as to appointments with suboxone certified providers (those are more in west metro, so if transportation is an issue, that may not be a viable option, although Medicaid members could get a ride set up).
Rule-outs
Must be a Medica member.
Different providers have different biases on this; things such as being perceived as inappropriately medication seeking would be an example; call to inquire.
