Canvas Health – Talk to an intake coordinator who will match you with a clinician who can get you the help they need in the location that’s most convenient. Canvas Health helps children, adolescents, adults and families who struggle with mental illness, substance use, unstable housing, trauma, and abuse.

Guild Community Access– provides assessment for housing consultation, SOAR, and Behavioral Health Home services. If Guild does not have the appropriate service for your needs Community Access will provide options for other services and supports. Housing Consultation is the first step in accessing Housing Stabilization services provided through Medical Assistance. SOAR (SSI/SSDI Outreach, Access and Recovery) assists people in applying for social security benefits. Behavioral Health Home is a health and wellness program that assists people in connecting to providers and resources in the community.

United Way 2-1-1 provides free and confidential health and human services information for people in Minnesota. We’re here 24 hours a day, 7 days a week to connect you with the resources and information you need.
Guild: offers integrated mental health, housing, and employment services to individuals living with mental illness and/or experiencing chronic homelessness. This includes a Community Access Team, a Behavioral Health Home health and wellness program, SOAR (SSI/SSDI Outreach, Access and Recovery), and Therapeutic Services/ Consultation.
