Services

  • Federally Qualified Health Center
  • Dental – 9 operatories
  • Primary Care – 3 days/week patient care through one physician; serves all ages; in person and telehealth services available; medication-assisted treatment through NPs
  • Behavioral Health – Therapy available via telehealth currently
  • MNSure Insurance Navigator/Community Connector – Care coordination, health insurance enrollment, financial counseling; Bilingual navigator available

  • Sliding Scale Fee Discount Program available – eligibility based on income and household size: https://odhc.org/financial-assistance/
  • Most private insurance plans are accepted; MA and MinnesotaCare are accepted

Hours: Monday through Friday, 8am – 5pm; phones open 30 minutes before open and after close; overnight on-call medical providers available through answering service

Mental Health Partial Hospitalization and Day Treatment Programs

  • Adult Partial Hospitalization Program (PHP): Structured, intensive short-term treatment in a group therapy environment, 8 chairs
    • Intake: Provide ROI and assessment; evaluate exclusionary criteria; call intake line or contact Shanna Buhr to triage and phone screen
  • Adolescent Partial Hospitalization (PHP): Ages 13–18 Higher acuity patients; lower level of care than inpatient | 2 hours school component followed by therapy treatment
    • Intake: Provide ROI, call intake line to schedule diagnostic assessment
  • Adult Day Treatment: Focused, therapeutic support and skill-building, 16 chairs (2 groups of 8)

Reflections Day Treatment

  • Child/adolescent day treatment
  • Chaska Program: Age 8 (2nd/3rd grade) through 5th/6th grade
  • Shakopee Program: 5/6th grade through 12th grade
  • Structured integrated model with educational classes provided by 288 Southwest Metro and intensive (primarily group) therapy and skill-building services

Intake

  • Provide ROI and documentation from parents if parents requesting referral
  • Provide collateral, medical history, behavior history, and school/educational information
  • For Allina PHP: fill out intake form

    • Integrated Health Center will be along a fixed bus route

  • Microtransit – On-demand rides from point A to point B
    • Book via phone up to 3 hours in advance
    • May pick up others along the route; does not guarantee what time an individual will arrive at destination
