Intake Process

Have access to: Name, Address, phone and insurance info if you have it. They can register by phone or in person.
Service Description
Mental health assessment and counseling provided by on-site licensed psychologists.
Rule-outs
To receive mental health services they need to already be a patient for medical and/or dental at United Family Health, or register for those services.
Additional Information
Flexible payment options, including sliding scale fees, are available to those who qualify.
UFM accepts Medicaid, Medicare, and over 400 other insurances. We also offer a sliding fee discount for the uninsured, and no one is denied access to any services.
